Whether due to hospitalization, diagnosis, or gradual decline, finding the care you or a loved one needs in order to remain at home is a challenge that no one looks forward to. CDM has dedicated the last 35+ years to helping individuals and families through this process. We have a variety of services designed to meet the individual circumstances of each client, of any age. We also have decades-long partnerships with other experts in Southwest Washington to help you explore funding choices, fix transportation challenges, and to solve whatever issues are keeping you or your loved one from remaining at home.
We have dedicated staff, caring volunteers, and decades of experience. As a 501(c)(3) non-profit agency, we have raised money to help those most in need. If a client can be safely served at home, we will find the solutions to make that happen. Please take a look at some of our programs here:
